In consideration of participation in our childcare programs, I agree that my likeness or the likeness of my child/ward may be photographed or videotaped and that any such image or video may be published and used to publicize or promote the Rogers Activity Center summer program online, in print, or in video without acknowledgment or compensation.

We hereby consent to any and all health care providers to provide care to my child for any necessary medical care, first aid, transportation as necessary as a result of injury or illness.

I agree to abide by the Rogers Activity Center's code of conduct. As a parent/guardian of the participant, I agree to adhere to the outlined policies and procedures set forth by the staff.

Participants may be subject to interviews by licensing staff, child maltreatment investigators, or law enforcement officials for the purpose of determining licensing compliance or for investigative purposes. Participant interviews do not require parental notice or consent.

Parents will be notified of the issue as well as all processes that have taken place to deter unacceptable or dangerous behavior. The child can continue to participate in the program with parents providing adult supervision (must have approved background and maltreatment screening) until which time Department of Education, parents, and SDC staff can determine a plan to prevent unacceptable or dangerous behavior.

I am aware of the Field Trip Program and I hereby authorize the Rogers Activity Center Staff and contracted transportation providers permission to transport my child to and from field trips.

Cancellation is due 10 days prior to that week's camp starting.

Late fees are assessed at 6:00 AM on Monday.

If payments are behind more than 1 week, my child can be unenrolled from the remainder of camp and re-enrollment is based on availability.

My initial payment is NON REFUNDABLE even if I cancel all weeks of enrollment.
